We’re delighted that you’ve chosen Nephew Physical Therapy. As a new patient, you won’t find much in the way of fine print here at Nephew PT--just a few forms and items we think will be helpful for you to know before your first appointment.
WELCOME LETTER
Prior to your first visit, you’ll save time by downloading and completing the forms below. Prefer to complete the forms in house? No problem. We simply ask that you plan to arrive15 minutes early to fill out a few pages of necessary information. If you’re under 18, we’ll need your guardian’s consent before we can treat you, so please be sure to complete the Minor Consent Form as well.

Here’s What to Expect On the day of your first appointment, you’ll be ready to roll when you arrive on time, carrying your insurance card(s) and your doctor’s referral (if applicable). You’ll want to be able to move with ease, so loose-fitting, comfortable clothing is best (no jeans, please!), and there are locker rooms on site for your convenience should you need to change.
So…with your forms, insurance card(s), referral and comfy clothes in hand, you’re probably thinking, "It’s that simple?" To that, we say, "Absolutely!"

What Happens After Therapy? So you’ve been to Nephew Physical Therapy, received treatment and been discharged on good behavior. Wait. What we meant to say was—and been discharged because YOU’RE FEELING BETTER and back on the go. We think that’s exciting—and we hope you do, too!
Now that you’ve graduated from your therapy program, we’d love to hear about your experience. Your opinion is VERY important to us, as we know we can only serve you and others better in the future by hearing from those have been in our care. Below you’ll find a link to our Discharge Packet. We ask that you print it out (hard copies are available at our office if you’d like one), complete it to the best of your ability, and return it to us while your Physical Therapy experience is still fresh to you.
